再び要望書を提出します

私は6年前の2010年に、当時の日本皮膚科学会アトピー性皮膚炎診療ガイドラインに、ステロイド外用剤による依存やリバウンドという副作用についての記述が無いことを問題視して、同志数名とともに学会に要望書を提出しました(→こちら)。
先日、新しいガイドラインが公表されましたが、いまだこの重要な副作用に関する警告の記述がありません(2016年版ガイドラインは→こちら)。
そこで、再び先回同様、要望書を提出したいと考えます。文案は下記の通りです(今後有志と詰めて文章を修正する可能性があります)。

謹啓、日頃各種学会活動ではお世話になっております。厚く御礼申し上げます。
私は6年前に、日本皮膚科学会が作成したアトピー性皮膚炎診療ガイドラインにステロイド外用剤の副作用である、依存(Steroid addiction)やリバウンドについての記載がないことを危惧して、これを追記修正すべきとの意見を、他の日本皮膚科学会会員数名とともに要望した者です(平成22年3月10日付け内容証明郵便にてご送付申し上げております)。
しかしながら、今回2016年の改訂においても、この点に関する記載がなく、この副作用によって苦しむ患者たちへの注意喚起がまったくなされていない点は誠に遺憾です。
J Am Acad Dermatolの2006 年1月号には、Adverse effects of topical glucocorticosteroids.という総説が掲載されており、ステロイド外用剤依存(Topical steroid addiction)は独立した副作用として記載されております(1)。また、2015年3月号にはこの問題に関するシステマティックレビューが掲載されました(2)。日本皮膚科学会の将来に禍根を残さないためにも、学会が真摯にこの問題に向き合い、ガイドラインの速やかな改訂または補足を追加される ことを、ここに要望いたします。

先回の要望書を提出したあとに、当たり前ですが、何か嫌がらせのような働きかけは、学会側からありませんでしたし、私の専門医も普通に更新しています。
また、前回もそうでしたが、このブログなどで、私以外の賛同者の個人名を出す予定はありません。
脱ステロイドを認識する皮膚科医が、この要望書に賛同して名前を連ねることは、将来において名誉でこそあれ、不利益を蒙るとは考えられません。
このように日本皮膚科学会会員有志が内部から異を唱えてアピールしていくことは、将来必ず何らかの実を結ぶと思います。
「ステロイド外用剤による依存やリバウンド現象は存在する」と薄々感じている現場の皮膚科医の皆さん。とくに若い方々に呼びかけます。
なぜその意見が封印され続けなければならないのでしょうか?
十年以上前に当時の学会の偉い先生方が、誤った認識の元に睨みをきかせて言論統制を行いました。
それにおびえてこの副作用の存在を口にすることすらタブー視して萎縮してしまった情けない世代の古くて弱い皮膚科医たちを、これからもあなた方はお手本にし続けるのだろうか?
依存やリバウンドというのは確かに機序も不明だし、基礎となる皮膚疾患の単なる悪化と紛らわしいです。
しかし患者の経過をよく観察している皮膚科医であるならば、これが単なる原疾患の悪化ではなさそうだということは感じる筈です。なぜならそのような画像認識に基づく直観的な分類こそが、皮膚科診断学の基本であるのですから。
アトピー性皮膚炎におけるステロイド外用剤依存やリバウンドの問題は、消えてなくなることはありません。いつかは直面せざるを得なくなる問題です。
先送りにすればするほど深刻化し、過去の罪は深くなります。日本皮膚科学会の将来のためにも、どうか要望書にご賛同ください。

TITLE : I submit a request again
I submitted a request that Japanese guideline for atopic dermatitis should contain the description about topical steroid addiction or rebound phenomenon as a side effect of topical corticosteroids with several comrades in 2010 (six years ago) to Japanese Dermatological Association .
A new guideline was published recently but there is no description of warning the important side effect.
I made up my mind to submit a request again similarly to the last time. The draft is as the following.

Dear sirs
Thank you for the various usual academic activities. I appreciate them very much.
I submitted a request that Japanese guideline for atopic dermatitis should contain the description about topical steroid addiction or rebound phenomenon as a side effect of topical corticosteroids with some other members of JDA six years ago.
However, it is a matter for regret that there is no description or warning about the problem for the patients suffering from this side effect in the revised version in 2016 also.
There is a review entitled “Adverse effects of topical glucocorticosteroids” in J Am Acad Dermatol published in Jan 2006 in which topical steroid addiction is classified as an independent side effect of topical corticosteroids(1). There is also a publication of systematic review about the illness in the same journal in Mar 2015(2). For not leaving the root of evil for the future of JDA, I request JDA to face the problem seriously and revise the guideline immediately or add a supplementation about the problem.

I ask to all members of JDA who are reading my blog. Why don’t you add your name in the above request letter? I am willing to revise the draft together with you. If you agree, please send an e-mail to me.
Not to mention, there has been no harassment from JDA after the last submission. My capacity as a JDA certificated specialist has been revised without any problem.
I will never publish your names without permission anywhere including my blog.
It must be honored in the future but never be at a disadvantage for a dermatologist who recognize the importance of topical steroid withdrawal to approve of the request and sign the name to it. I believe it must bear fruit in the future that some interested members of JDA voice from inside a different opinion to the guideline.
I address it to dermatological practitioners and young doctors especially who have become vaguely aware of the existence of topical steroid addiction or rebound phenomenon .Why should the opinion continue to be sealed?
Authorities of the academy controlled of free speech more than 10 years ago under the wrong recognition. Will you continue to follow in the footsteps of the old and weak dermatologists who have miserably boggled to speak of the existence of this side effect fearing such authorities?
The mechanism of topical steroid addiction or rebound phenomenon has not been elucidated yet. They are really confusing with the simple aggravation of the original illness existing at the background.
However, dermatologists who observe the procedure of illness well must have recognized that it is different from the simple aggravation of the original illness. Such an intrinsic classification method based on the image recognition is very base of dermatological diagnosis.
The problem of the topical steroid addiction or rebound phenomenon will never disappear. All the dermatologists must face it finally. The later we put off the issue, the worse it becomes and the more serious the crime of us dermatologists becomes. For the future of JDA, I call for your signatures to the request.
